• Child’s Birth Certificate
• Parents’ Holy Cross Registration Form or permission letter from church where they are registered
• Godparent’s ID
• Godparent’s Sponsor Form
• Godparent’s Sponsor Card if not a member here.
• Single Godparent or Christian Witness—Baptism Cert.
• Married Godparent or Christian Witness— Marriage Certificate from Catholic Church

• Complete Baptism Registration form
• Copy of Child’s Birth Certificate
